Self-Reporting Crashes

Starting January 2020, PPD will no longer be responding to most parking lot crashes. If you are involved in a crash that does not involve the following, you are able to fill out a crash report and submit it to the Florida Department of Highway Safety and Motor Vehicles. (Reference Florida Statute 316.061 and Florida Statute 316.062.)

  • A crash with injury;
  • A crash with fatality;
  • A hit and run crash;
  • A DUI crash;
  • A crash which requires a wrecker to remove the vehicle;
  • A crash with a commercial motor vehicle; or
  • A crash where apparent damage is at least $500.00.

If your crash involves any of the items listed above, please stay at the scene of the crash and call (850)435-1845 for
non-emergencies and 911 for emergencies.

  1. Please complete all applicable fields within the linked form, including signature and date. Instructions can be found on page 2 of the form.
  2. Once completed, a copy of the "Driver Self Report of Traffic Crash" can be emailed to SelfReportCrashes@flhsmv.gov

  1. Please complete all applicable areas within the form. Customer must sign the report.
  2. Once complete, mail a copy of the report to:

  3. Please keep a copy of the submitted report for your records and insurance purposes.
